Our Intake Coordinators are available by phone (1-205-678-4373) or this Online Contact Form to answer questions about eating disorders, recovery, and our treatment programs. As a courtesy to potential clients, out Intake Coordinators can check insurance benefits if you complete the Inquiry Application through our client portal.
Potential clients may complete clinical intake paperwork online or over the phone; potential clients can expect to provide relevant personal, medical, social, and psychiatric information. With potential clients' permission, members of the Magnolia Creek team may consult family members, referring clinicians and/or physicians, in order to obtain additional, pertinent information. Potential clients must also submit recent lab work (from a physician) prior to admission. When Magnolia Creek is at capacity, potential clients may submit a deposit to reserve a spot on the program's waiting list.
To complete the clinical intake paperwork online click here to create a new account. Make sure to write your password down somewhere safe. Once the account is set up you will complete the Intake Application. The completed intake will be used to provide the clinical team with information on how we may be able to best support you, so please be thorough while completing the intake. Once the Intake Application is complete, we will contact you regarding next steps. Please don't hesitate to reach out with questions at any point throughout this process.
Clients are assessed by our medical, dietetic, and therapeutic staff on admission day. Staff is available to transport arriving clients.
