We realize that as the outpatient professional, you typically have a long-term, trusting relationship with your client and can offer insights and guidance to facilitate her treatment at the residential/PHP level.
Our goal is to provide the highest quality of care in a personal setting.
Upon arrival to our facility, your client will be assigned a primary therapist, who will meet with your client at least four times a week at the residential level and three times a week at the partial level.
We are committed to facilitating the transition to and from your level of care.
The primary therapist will call you within the first week to obtain collateral information from you to assist in goal planning. You will also receive a copy of the bi-monthly treatment plan by fax or email, as well as a weekly update (by phone or email) as to your client’s progress.
While your client is in treatment with us, we would also like to fly you in for a complimentary visit during a scheduled Family Weekend to meet with our treatment team, tour the facility, and meet with your client to plan for transition. The primary therapist or clinical program manager will call you for aftercare planning close to your client’s discharge, and you will receive a discharge summary at one week post-discharge.
